Report an allegation of misconduct on a Law Enforcement and Investigations employee

The USDA Forest Service Law Enforcement and Investigations (LEI) Office of Professional Responsibility is responsible for conducting investigations alleging criminal acts and/or misconduct involving LEI personnel.

Individuals who wish to file a complaint against LEI personnel with USDA Forest Service Law Enforcement and Investigations can use the online LEI employee alleged misconduct complaint form. Individuals may submit an anonymous complaint.

When filing an online complaint please include as much information as possible to help ensure a thorough investigation is conducted, such as;

Describe the incident, including the date, time and location.
Provide the LEI employee’s name, badge number and physical description.
Identify any witnesses including names, addresses, and telephone numbers.
Describe the vehicle, including the license plate number and state.
If applicable, please provide any supporting documentation when submitting your online complaint (i.e., pictures, witness statements, copy of incident report or violation notice).
